Daily Status Report Generatiuon Form

Don't use this form, it doesn't work yet. This form should be filled in to submit the daily SLD status report to DUCSNEWS. It should initially be filled in with yesterday's report. Just update the fields that need changing and press SUBMIT to send the new report to DUCSNEWS. Don't forget to also update the phone message.
